چکیده

Abstract Background Low-density lipoprotein (LDL-C) lowering is imperative in cardiovascular disease prevention. Effectively translating the evidence for LDL-C to maximize clinical and public health benefits depends on availability of accurate results from laboratories guide therapy. Furthermore, prior work has raised possibility that cholesterol esterase transfer protein (CETP) inhibition could interfere with assessment LDL-C. Purpose We aimed compare accuracy three clinically implemented equations a trial CETP inhibition. Methods Men women aged 18–75 years dyslipidaemia were recruited 17 sites Netherlands Denmark. Patients randomly assigned one nine groups using various combinations inhibitor TA-8995, statin therapy, placebo. In pooled measurements over 12 weeks, we calculated by Friedewald, Martin/Hopkins, Sampson equations, compared values preparative ultracentrifugation (PUC) as reference measure (also known “beta quantification”). Based literature guidelines, examined correct classifications across 1.81 mmol/L cutpoint subgroup patients triglycerides 1.69–4.51 mmol/L. Results The analysis included 242 contributing 921 observations. full distributions differences between estimates PUC are shown Figure 1. Overall median small: 0.00 (25th, 75th: −0.10, 0.08) mmol/L; 0.02 (−0.08, 0.10) Sampson, 0.05 (−0.03, 0.15) estimated <1.81 mmol/L, Friedewald equation underestimated difference versus −0.25 (−0.33, −0.10) whereas Martin/Hopkins corrected this issue showed tendency towards underestimation −0.06 (−0.13, 0.00) triglyceride levels 2 shows proportion classified < or ≥1.81 correctly when PUC. All >95% However, observations was 71.4% 100.0% 93.1% Sampson. Conclusion Europeans dyslipidaemia, our improved contemporary equation, particularly equation. High seen individuals treated even context low high levels. Funding Acknowledgement Type funding sources: Private company. Main source(s): Dezima NewAmsterdam Pharma

Clinical significance of high-density lipoprotein cholesterol in patients with low low-density lipoprotein cholesterol.

OBJECTIVES We sought to evaluate the significance of high-density lipoprotein cholesterol (HDL-C) in the context of low low-density lipoprotein cholesterol (LDL-C). BACKGROUND Earlier studies support an inverse correlation between circulating HDL-C and coronary risk in patients with normal or elevated LDL-C. METHODS This study involved 4,188 patients attending the Palo Alto Veterans Adminis...

International clinical practice guidelines on dyslipidemia emphasize the importance of matching the use and intensity of statin therapy with the absolute risk of atherosclerotic cardiovascular disease (CVD). Indeed, independent of the baseline level of dyslipidemia, patients who have entered the intervention arms of randomized clinical trials of statin therapy with similar absolute risks of CVD...

Beyond Low-Density Lipoprotein Cholesterol

Recent clinical trials in patients with coronary artery disease (CAD) provide evidence that low-density lipoprotein cholesterol (LDL-C) levels should be lowered even further to prevent recurrent CAD. However, despite more aggressive interventions for lowering LDL-C levels, the majority of CAD events go undeterred, perhaps related to the fact that intervention was not started earlier in life or ...

The levels of plasma low density lipoprotein are independent of cholesterol ester transfer protein in fish-oil fed F1B hamsters

BACKGROUND: Cholesterol ester transfer protein (CETP) plays a major role in regulating the levels of LDL- and HDL-cholesterol. We previously observed a fish-oil-induced elevation of low-density lipoprotein (LDL)-and very-low-density lipoprotein (VLDL)-cholesterol concentrations and a decrease in high-density lipoprotein (HDL)-cholesterol concentration in F1B hamsters. The molecular mechanism/s ...

Cholesteryl ester transfer protein inhibition: effect on reverse cholesterol transport?

Statins, inhibitors of the key enzyme in the biosynthesis of cholesterol (ie, 3-hydroxy-3-methylglutaryl [HMG]coenzyme A [CoA] reductase), are widely used as the prevailing strategy to combat atherosclerosis through reducing LDL cholesterol levels. Large-scale clinical trials have shown that statins markedly reduce coronary events.1,2 A meta-analysis of 22 studies enrolling nearly 70 000 indivi...
